Iron in Childhood: Why Age, Diet and Risk Factors Matter

Iron needs and iron-deficiency risk change across childhood. In 2026, the American Academy of Pediatrics (AAP) updated its clinical guidance to cover prevention, screening, diagnosis and treatment from infancy through adolescence.

Infancy

Feeding pattern and birth history matter. Current AAP guidance addresses iron supplementation for preterm infants and for exclusively breastfed infants, along with the transition to iron-containing complementary foods. These decisions should be based on age, feeding history and individual risk rather than inferred from a multivitamin label.

Toddlers and growing children

Dietary iron remains important. Foods such as meats, beans, lentils and iron-fortified cereals can contribute iron. Excessive milk intake in young children can displace iron-rich foods, which is one reason dietary history matters when risk is assessed.

Adolescence

Risk assessment remains important in teens. Current AAP guidance highlights heavy menstrual bleeding after menarche as an important factor, along with other circumstances that can increase iron needs or reduce intake. Symptoms alone cannot diagnose iron deficiency; clinical evaluation and appropriate testing matter.
